Team Nurse: Outpatient Primary Care & Care Coordination

Beth Israel Lahey Health’s Team Nurse supports outpatient primary care and family medicine clinics, reporting to the Practice Manager and Senior Director of Nursing. The RN/LPN provides high-quality, direct patient care in a team-based model and serves as a clinical resource for the practice.

The Nurse educates and trains Medical Assistant staff, coordinates patient-centered care, and supports transitions between care levels while promoting safety and PHI protection.
